Online forms for the 2026-27 school year will not be available until 4/1/26

Physicals:
All incoming 7-12th grade students for the 2025-26 school year involved in athletics, cheer, dance, marching band and ROTC are required to have an annual physical. All completed physicals can be sent via pdf format to [email protected] All physicals for the 2026-2027 school year must be dated on or after February 1, 2026.
Athletic physicals for the district should be completed on the official UIL physical form

Saturday, Apr 18, 2026@ MHS cafeteria from 8a-11a.
Saturday, May 2, 2026@ Methodist Midlothian clinic 8a-11a.

EKG Information: Methodist will provide free EKG’s in the months of April and May of each year.

Frequently Asked Questions
1. What physical or online forms does my child need in order to participate in athletics/pre-athletics/band/cheer, etc?
- Incoming 6th graders interested in pre-athletics will not need an athletic physical.
- Incoming 7-12th graders interested in athletics will need to complete the UIL Medical History, Physical Examination form AND Online forms.
- Incoming 7-12th graders interested in cheer/drill team or 9-12th marching band will need to complete the UIL Medical History and Physical Examination form.
2. How often will my child need to get a physical?
Midlothian ISD policy states that every student athlete is required to have an annual Pre-Participation Physical Examination performed prior to the beginning of the athletic season. This physical examination must be done at the student-athlete’s own expense. In accordance with UIL rules, no student athlete will be allowed to participate in any activity/practice before, during, or after school, (both in-season and out-of-season) or games/matches without a completed current physical form on file.
3. My child gets their annual check-up/physical with our primary care doctor every year. Will this be accepted by MISD Athletics?
Yes; however, there are a few guidelines to follow. The current and most up to date UIL Medical History and Pre-Participation Physical Examination form must be used, with no exceptions. This can be found on the MISD Athletics website under the "Online Forms" section. These forms must be filled out entirely and signed by a physician, parent/guardian and student athlete. Incomplete forms will not be accepted. Physicals must be administered after April 1, 2025 to be accepted for the 2025-26 school year.
3. Can I turn in my child's physical over the summer break?
Yes. Whether your child is an incoming middle school or high school athlete, you do not have to wait for school to start up before turning in your child's physical. You have two options:

We ask that you refrain from submitting your child's physical to the middle schools during summer break as it is more likely to get lost in the shuffle.
